A self-service form collects a person's details, verifies their email, and prepares an agreement for review and signing.
Prepare your template
Save a template with the document content and recipient roles first. Add document details for values the requester will enter.
For generated Word or in-app documents, choose Use document placeholders or Add a signing page on Prepare. Forms cannot use generated templates with manually positioned PDF fields.
Create the form
Open Forms and select Create a form.
Enter a Form name, choose the Saved template, and write an Introduction.
Under People, choose the role for the Person filling out the form. Their verified email fills this role.
Enter the name and email for every additional recipient. These recipients stay fixed and are not shown on the public form.
Under Requested details, choose which document details to ask for. Set each question label, answer format, and Required option.
Select Preview questions to inspect the form, then Save form.
A detail you leave out keeps its saved template value. If the template has no merge details, the form asks for the requester's name and email.
Choose the template and identify the person who fills out the form.
Publish and share
Select Preview form to review the saved form. The preview creates no agreement and sends no email.
Select Publish, then Publish form. Copy the public link immediately. RakeSign shows it only when you publish.
Anyone with the link can request an agreement after verifying their email.
Preview the questions before you publish your form.
Change or stop a form
Saving changes disables the current public link. Publish the updated form and share its new link.
Create a new link also disables the previous link. Revoke link stops new requests through the current link. Agreements already sent remain unaffected.
